Request an Investigation

CLICK HERE to open our online Investigation request form. When you complete the form and click "SUBMIT" the form will automatically be sent to our Case Manager for review. Please be sure to include your contact details and as much information as you can to assist our Case Manager in reviewing your request.

G.H.O.S.T. Highlights

ALL requests are taken very seriously. Once received, our Case Manager will contact you as soon as possible to discuss your request and schedule a personal interview.

The personal interview will take approximately 1-2 hours depending on the circumstances you described in your request. During the interview, you will be asked to sign an "Authorization form" allowing us to investigate the location you requested. You must have the authority to sign such authorization. We will not investigate any location without proper signed authorization to do so.
